Navigator Program
The Greater Trail Hospice Society provides this special program to journey with those who have been diagnosed with life-limiting chronic illness, helping the person and their family to navigate through changes and challenges, connect with resources, learn about their conditions, and anticipate future needs.
The Volunteers who are a part of this program have undergone additional training and are committed to providing confidential compassionate care through active listening, assisting with navigating the system, and providing resource materials and information.
Greater Trail Hospice is partnered with West Kootenay Nav-CARE.
Nav-CARE Dementia Program
Trail Hospice, Better at Home and Columbia Seniors Wellness Society are teaming up to support people living with dementia in the Greater Trail area, including Rossland, Warfield, Genelle and the Beaver Valley.
We are organizing dementia-specific navigation training for volunteers to work with people living with dementia and their caregivers.
We will be organizing initiatives to bring compassion and inclusion for people living with dementia and to challenge stigmas about this misunderstood disease.
